Medical Necessity 

Establishing medical necessity it the first step in third party reimbursement. Justifying care provided by presenting the appropriate facts. Payers require knowledge of the severity of the patients complaint or condition and all pertinent signs, symptoms, complaints, or background facts describing the reason for the encounter. These facts must be substantiated by the patient medical record.
